This form is used when Students or Parents are requesting that the division share personal information, such as birthdate, grade or program placement, academic standing, or other student record information; or when an individual or organization requests information about a student of Northern Lights Public Schools.

The form is available for staff to download on the Forms page of Staff Connect. Parents/students or outside agencies who require a copy of the form can request one from the school the student attends.
